Ketoconazole Indications

Ketoconazole, a synthetic imidazole dioxolane derivative, belongs to the antifungal class of drugs. Ketoconazole is indicated for the treatment of infections like fungal, ringworm and yeast and against Cushing’s disease (hyperadrenocorticism). The drug is efficient against serious fungal infections like coccidoidomycosis, blastomycosis and cryptococcosis, which affect lymph nodes, eyes, bone, skin and lungs.

How Ketoconazole works

Ketoconazole is fungicidal or fungistatic depending on concentrations. It inhibits the conversion of Lanosterol to 14 demethyl Lanosterol by inhibiting the cytochrome P450 enzyme 14 alpha demethylase and impair ergosterol synthesis. This will alter cell membrane and its permeability. This will results in loss of intracellular components. It also inhibits biosynthesis of triglycerides and phospholipids by fungi. It also inhibits several enzymes (e.g. Oxidative and peroxidative enzymes) which are involved in detoxification process. This will leads to cellular necrosis.

Ketoconazole Precautions

The drug is not recommended for use in pregnant or nursing animals. The medication should not be used within 2 hours of an antacid and with cisapride, astemizole or terfenadine. Extreme caution is required when used with phenobarbital, cyclosporine, theophylline, phenytoin, and anticoagulants such as warfarin.
